This form is now authorized for local reproduction. You can request camera copy of SF 527 from General Services Administration (CARM), Attn: Barbara Williams, (202) 501-0581. This form also is revised to: 1. Delete the line that extended from the caption to the arrow in captions ``Date'' and ``Indicate Side Tested'' on both sides of the form. 2. To delete ``grade; SSAN; rank; rate;'' from ``PATIENT'S IDENTIFICATION'' item and replace with ``ID no. (SSN or other);''.
