Most hospitals that offer tissue donations work with a regional consortium. The consortium usually has their own form. Therefore SF 523B is being cancelled and replaced [[Page 4168]] by OF 523B allowing individual hospitals to use the consortium forms. The optional form is authorized for local reproduction.
