You can request camera copy of SF 526 from General Services Administration (CARM), Attn.: Barbara Williams, (202) 501- 0581. This form also is revised to: To delete ``grade'' and ``date'' from ``PATIENT'S IDENTIFICATION'' item and replace with ``ID No. (SSN or other);''.
