This information collection request (ICR) is entitled "United States Coast Guard Academy Application and Supplemental Forms" and is assigned Office of Management and Budget (OMB) control number 1625-0004. Before submitting this ICR to OMB for reinstatement, the Coast Guard is inviting your comments on our request.
