Department of Labor Submits Information Collection Request to OMB for Review and Approval

The Department of Labor is submitting an information collection request to the Office of Management and Budget for review and approval, as required by the Paperwork Reduction Act of 1995, to use the revised form titled 'Operator Controversion, Operator Response, Operator Response to Schedule for Submission of Additional Evidence, and Operator Response to Notice of Claim'.
