Agency Seeks Public Comment on New Information Collection Request

The Office of the Secretary at the Department of Health and Human Services is requesting public comment on a new information collection request, which must be submitted to the Office of Management and Budget by a certain date, in order to comply with the Paperwork Reduction Act of 1995.
