New External Review Process for Health Insurance Plans Under the Affordable Care Act

The Affordable Care Act requires health insurance plans and issuers to have effective internal claims and appeals processes, as well as external review processes, which must comply with either a state process or a federal process meeting HHS standards, starting from plan years beginning on or after September 23, 2010.
