Submission for OMB Review; Request for Comments on a Revised Information Collection

The Office of Personnel Management (OPM) has submitted a request to the Office of Management and Budget (OMB) for public comments on a revised information collection form, the Civil Service Retirement System (CSRS) Survivor Annuitant Express Pay Application for Death Benefits (Form RI 25-51). The form will be used to pay death benefits to the widow(er) of an annuitant, with approximately 34,800 forms completed annually.
