30-Day Notice of Proposed Information Collection: Three (3) Passport Services Information Collections

The U.S. Department of State is seeking public comment on three passport services information collections, including a birth affidavit, a change of name affidavit, and a statement of non-receipt of a U.S. passport, as part of its submission to the Office of Management and Budget for approval under the Paperwork Reduction Act of 1995.
