Submission for OMB Review; Comment Request for a Revised Information Collection: OPM Form 1644

The Office of Personnel Management (OPM) has submitted a request to the Office of Management and Budget (OMB) for a revised information collection, OPM Form 1644, to verify child care providers meet statutory requirements. The form is used to license and regulate child care providers and is expected to be completed by approximately 2000 providers annually.
