U.S. Department of State Seeks OMB Approval for Revised Passport Witness Affidavit Form

The Department of State is requesting a 60-day public comment period for a revised passport witness affidavit form, DS-71, which is expected to collect data from approximately 140,000 individuals or households, requiring an estimated 11,700 hours of response time.
