Department of State Requests Approval for Passport Information Collection

The Department of State has submitted a request to the Office of Management and Budget for approval to extend a currently approved information collection, specifically the Statement Regarding Lost or Stolen Passport form (DS-64), which is used by individuals or households on occasion. Comments must be submitted within 60 days to the Office of Management and Budget.
