Office of Personnel Management Requests Review of Revised Information Collection Forms

The Office of Personnel Management is seeking review of revised forms SF 2802, SF 2802-B, and RI 36-7, which are used to process refunds of retirement contributions from the Civil Service Retirement and Disability Fund. The forms are used to collect information about applicants' marital status and are completed by approximately 35,000 employees annually.
