Medicare, Medicaid, and Children's Health Insurance Programs: 2011 Provider Enrollment Application Fee

The Centers for Medicare & Medicaid Services (CMS) has announced a $505 application fee for institutional providers enrolling in Medicare, revalidating their enrollment, or adding a new Medicare practice location, with a deadline of December 31, 2011, and March 25, 2011 for prospective or re-enrolling Medicaid or CHIP providers.
