U.S. Department of State Seeks OMB Approval for Affidavit of Relationship Information Collection

The Department of State is requesting public comment on a proposed information collection, an Affidavit of Relationship, in order to obtain Office of Management and Budget approval, as required by the Paperwork Reduction Act of 1995.
