Department of Labor Submits Information Collection Request for Student Death Benefit Application to OMB for Review and Approval

The Department of Labor is requesting the Office of Management and Budget to review and approve the Office of Workers' Compensation Programs' information collection request for the 'Application for Continuation of Death Benefit for Student' form, which is used to continue death benefits for students, without any changes, in accordance with the Paperwork Reduction Act of 1995.
