Common Formats for Patient Safety Data Collection and Event Reporting

The U.S. Department of Health and Human Services' Agency for Healthcare Research and Quality (AHRQ) is releasing a new beta version of a standardized format for collecting and reporting patient safety data, specifically for Venous Thromboembolism (VTE), in an effort to improve healthcare quality and safety.
