Medicare Program; Independence at Home Demonstration Program

This notice creates a new demonstration program for chronically ill Medicare beneficiaries to test a payment incentive and service delivery system that utilizes physician and nurse practitioner directed home-based primary care teams aimed at improving health outcomes and reducing expenditures, beginning December 21, 2011.

This notice invites comment on a proposed information collection project titled Public Health Crisis Response (PHCR) Cooperative Agreement.
