DOL Submits Information Collection Request for Health Insurance Claim Form to OMB for Review

The Department of Labor is seeking OMB approval to revise its Health Insurance Claim Form, Form OWCP-1500, as part of its Office of Workers' Compensation Programs' information collection activities, in accordance with the Paperwork Reduction Act of 1995.
