Department of State Submits 30-Day Notice for Revised Birth Affidavit Information Collection

The Department of State has submitted a revised information collection request to the Office of Management and Budget for approval, which will require individuals or households to submit a birth affidavit, known as form DS-10, to obtain or retain a benefit.
