U.S. Department of Health and Human Services Proposes New Data Collection for Serious Medical Procedure Requests

The Office of Refugee Resettlement, part of the Administration for Children and Families within the U.S. Department of Health and Human Services, is proposing to collect data for a new data collection, the Serious Medical Procedure Request (SMR) Form, to gather information on medical procedures related to refugee resettlement.
