Medicare, Medicaid, and Children's Health Insurance Programs; Provider Enrollment Application Fee Amount for Calendar Year 2020

The Centers for Medicare & Medicaid Services (CMS) is requiring a $595.00 application fee for institutional providers enrolling in Medicare, Medicaid, or the Children's Health Insurance Program (CHIP) for the 2020 calendar year. This fee applies to new enrollments, revalidations, or adding new locations, and must be submitted between January 1, 2020, and December 31, 2020.
