ACF Requests Three-Year Extension of Child Welfare Capacity Building Collaborative Forms

The Administration for Children and Families (ACF) is requesting a three-year extension of several forms used in the Child Welfare Capacity Building Collaborative evaluation, including satisfaction surveys and service-specific feedback forms. The request includes minor changes to webinar and online learning registration forms and excludes three original instruments.
