U.S. Department of State Seeks OMB Approval for Revised Passport Application Form

The Department of State is requesting a 60-day public comment period for a revised passport application form, DS-4085, which will allow individuals or households to request additional visa pages or miscellaneous passport services, and is estimated to require approximately 48,700 hours of annual response time.
