U.S. Department of State Submits Request for Approval of Revised Passport Form DS-4085

The Department of State has submitted a request to the Office of Management and Budget for approval of a revised passport form, DS-4085, which will allow individuals to request additional visa pages or miscellaneous passport services, with an estimated 146,000 respondents per year expected to complete the form.
