U.S. Department of State Submits 30-Day Notice for Passport Application Form DS-11

The U.S. Department of State has submitted a request to the Office of Management and Budget for approval to revise its current passport application form, DS-11, which is expected to be completed by approximately 12.5 million individuals or households annually.
