U.S. Department of State Submits 30-Day Notice for Passport Application Form DS-5504

The U.S. Department of State has submitted a request to the Office of Management and Budget for approval of a revised passport application form, DS-5504, which will be used for name changes, data corrections, and limited passport book replacements, affecting approximately 181,000 individuals or households annually.
