Medicare Program; Start-Up Funding in Support of the Vermont All-Payer Accountable Care Organization (ACO) Model-Cooperative Agreement

The Medicare program is providing funding to support the Vermont Agency of Human Services in implementing the Vermont All-Payer Accountable Care Organization (ACO) Model, with the goal of improving care coordination and collaboration among healthcare providers in the state.
