FEMA Submits Information Collection Form for OMB Review and Clearance

The Federal Emergency Management Agency (FEMA) is requesting public comment on a new information collection form, which will be submitted to the Office of Management and Budget for review and clearance under the Paperwork Reduction Act of 1995.
