Department of Health and Human Services Submits Information Collection Request to OMB for Review and Approval

The Office of the Secretary at the Department of Health and Human Services is requesting public comment on a revision to an information collection form, scheduled to expire on November 30, 2015, in compliance with the Paperwork Reduction Act of 1995.
