OPM Seeks Public Comment on Extension of Disability Retirement Application Form

The Office of Personnel Management is requesting public feedback on an extension of the currently approved form SF 3112, 3206-0228, which supports disability retirement applications for CSRS/FERS employees, as required by the Paperwork Reduction Act of 1995.
