Rail Safety Investigation Reveals Failure to Address Defect Leading to Derailment
A US derailment in West Virginia on February 16, 2015, was caused by a broken rail resulting from an internal rail defect, specifically a vertical split head. The Federal Railroad Administration (FRA) believes that better understanding of rail flaw conditions, including rough rail surface conditions, could have prevented the derailment if the track owner had taken prompt remedial action.
