Request for Comments on Information Collection Request

The U.S. government is seeking public comments on an information collection request that was previously approved, in accordance with the Paperwork Reduction Act of 1995. This request was initially published in the Federal Register on April 7, 2015, and is now being forwarded to the Office of Management and Budget for review.
