Proposed Extension of Information Collection Request for Form 14145
The Internal Revenue Service (IRS) is seeking public comments on a proposed extension of its information collection request for Form 14145, the IRS Applicant Contact Card, as part of its efforts to reduce paperwork and respondent burden, in compliance with the Paperwork Reduction Act of 1995.
