Request for Comment on Draft Vaccines Adverse Event Reporting System (VAERS) 2.0 Form

The Centers for Disease Control and Prevention (CDC) is seeking public input on a proposed update to the Vaccines Adverse Event Reporting System (VAERS) 2.0 form, which will replace the current VAERS-1 form. The CDC and the U.S. Food and Drug Administration (FDA) co-administer VAERS, a system that tracks adverse events following vaccination.
