Medicare, Medicaid, and Children's Health Insurance Programs; Provider Enrollment Application Fee Amount for Calendar Year 2013

The Centers for Medicare & Medicaid Services (CMS) is requiring a $532.00 application fee for institutional providers enrolling in Medicare, Medicaid, or the Children's Health Insurance Program (CHIP) for the 2013 calendar year. This fee applies to new enrollments, revalidation, or adding a new practice location, and must be submitted between January 1, 2013, and December 31, 2013.
