U.S. Department of State Requests OMB Approval for Employee Self-Certification Form

The Department of State is seeking public comment on a proposed information collection form, the Employee Self-Certification and Ability To Perform in Emergencies (ESCAPE) Posts, Pre-Deployment Physical Exam Acknowledgement Form, in order to obtain Office of Management and Budget (OMB) approval.
