ACF Office of Refugee Resettlement Requests Update to Unaccompanied Alien Child Health Forms

The Administration for Children and Families is requesting to restructure and revise the Serious Medical Procedure Request Form and other related health forms to improve transparency, reduce burden, and enhance data quality, while also ensuring alignment with Office of Refugee Resettlement requirements and continuity of care with the Department of Homeland Security.
