VA Form 10-0048: Information Collection Under OMB Review for Former POW Medical History

The Department of Veterans Affairs is submitting a collection of information to the Office of Management and Budget for review and comment, in compliance with the Paperwork Reduction Act of 1995. The Veterans Health Administration will collect medical history information from former prisoners of war.
