Office of Personnel Management Requests Review of Revised Information Collection Forms

The Office of Personnel Management has submitted a request to the Office of Management and Budget for review of two revised information collection forms, Standard Form 2803 and Standard Form 3108, used by former Federal employees to make payment for Federal service. The forms are expected to be completed by approximately 150 individuals annually, taking around 30 minutes each.
