Office of Personnel Management Requests Extension Without Change for SF 2809 Health Benefit Election Form

The Office of Personnel Management has submitted a request to the Office of Management and Budget to extend the use of the SF 2809 Health Benefit Election Form without any changes, which is used by federal employees and certain annuitants to enroll in health benefits, with approximately 18,000 forms completed annually.
