Comment Request for Review of Revised Information Collection: Standard Form 2808

The Office of Personnel Management (OPM) has submitted a request to the Office of Management and Budget (OMB) for review of a revised information collection, Standard Form 2808, which is used by Civil Service Retirement System (CSRS) beneficiaries to designate a beneficiary for lump sum payments in the event of death, with approximately 2,000 forms completed annually.
