OPM Requests Extension of Information Collection for OPM Form 1530

The Office of Personnel Management (OPM) is seeking to extend the approval of its OPM Form 1530, used to collect health information from annuitants, due to the Paperwork Reduction Act of 1995. The form is expected to be completed by approximately 500 individuals annually, taking around 90 minutes to complete.
