Submission for OMB Review: Request for Review of Revised Information Collection for SF 2802 and SF 2802A Forms

The Office of Personnel Management (OPM) has submitted a request to the Office of Management and Budget (OMB) for review of revised information collection forms SF 2802 and SF 2802A, used for refund of retirement deductions. The forms are used to comply with legal requirements and are completed by approximately 3,741 SF 2802 and 3,389 SF 2802A forms annually.
