FDA Requests Public Comment on Proposed MedWatch Reporting Forms

The Food and Drug Administration (FDA) is seeking public input on the proposed expiration of MedWatch Forms 3500 and 3500A, used to report adverse events related to FDA-regulated products, by October 31, 2008.
