Department of State Submits 30-Day Notice for Revised Affidavit of Identifying Witness Form

The Department of State has submitted a 30-day notice to the Office of Management and Budget for approval of a revised Affidavit of Identifying Witness form, which will be used to collect information from individuals or households, with an estimated 163,300 respondents and a total estimated burden of 13,608 hours.
