Office of Personnel Management Requests Review of Existing Information Collection for Disability Retirement Applications

The Office of Personnel Management (OPM) is seeking review of its Standard Form 3112, which collects information from applicants for disability retirement, in accordance with the Paperwork Reduction Act of 1995. Comments are invited on the necessity and practical utility of the information collection.
