OPM Seeks Review of New Medical Examination Form

The U.S. Office of Personnel Management (OPM) is requesting review of a new information collection document, Optional Form XX, Certificate of Medical Examination, to replace the outdated SF-78 form. The new form will collect medical information from individuals in positions requiring physical fitness testing and medical examinations.
