Submission for OMB Review: Comment Request for Reclearance of a Revised Information Collection: SF 3102

The Office of Personnel Management (OPM) is seeking public comment on a revised information collection, SF 3102, which is used by federal employees to designate beneficiaries for lump sum payments in the event of death. The OPM has submitted a request for reclearance to the Office of Management and Budget.
