U.S. Department of State Seeks OMB Approval for Revised Birth Affidavit Form

The Department of State is requesting a 60-day public comment period for a revised birth affidavit form, DS-10, which is expected to collect information from 154,850 individuals or households annually, with an estimated total burden of 38,713 hours per year.
